The uniform format of the summaries includes: (1) the name of the employer organization; (2) a summary statement of the purpose of the program; (3) the issue(s) on which the program is focused; (4) business type; (5) program setting; (6) the year in which program was initiated; (6) source and level of funding; (7) the name, address, and telephone number of a contact person; (8) a summary of the program's history; (9) a list of primary program activities; and (10) information on the impact of the program.
