This pilot study focused on care transitions for older adults with multiple chronic conditions. Using a panel of experts, this qualitative pilot study was designed to test the usability of a web-based community care plan as a means of improving care transitions across the continuum of care. Core hand-off issues (care transitions) impacting hospitalization rates of the chronically ill were identified as part of the literature review. A Webinar described an electronic tool which was designed to mitigate problems during transitions of care. Input was gathered from an expert panel regarding the potential usefulness of the tool. Key areas addressed by the tool were identified as: sense of value/meaning, patient activation, care coordination, and accountability. While this study's results may not be transferable to all communities, the electronic community care record provides a platform for communication with the local professionals responsible for the care of seniors in their communities.
